Goh Chok Tong Enable Awards (Promise)
Nomination Form
The Goh Chok Tong Enable Awards, an initiative by Goh Chok Tong Enable Fund (GCTEF), was launched on 15 January 2019. The founding of the Awards was inspired by Emeritus Senior Minister Goh Chok Tong, who is the Patron of GCTEF.
It aims to recognise the achievements of persons with disabilities, and to motivate those with promise and potential to go the distance with their endeavours.
Award Details
- Objective: To encourage persons with disabilities who have shown promise and potential to pursue greater heights in their areas of talent / skills / abilities and willingness to serve the community.

Eligibility Criteria:
- Nominees must be Singapore Citizens or Permanent Residents, 12 years of age and older, with a permanent disability.
- Nominees must demonstrate one or more of the following criteria:
Resilience Exhibit courage and perseverance to overcome barriers in life. Character & Community Service Of good moral character; have begun their journey of community service. Performance & Potential Shown evidence of talent / skills / abilities, and have shown aptitude to hone them further, relative to their disabilities.

- All sections of the Nomination Form must be completed online.

The following supporting documents must be submitted online together with the completed Nomination Form by Tuesday, 23 June 2026:
- A clear copy of nominee's passport sized photo (maximum of 5MB in JPG / JPEG format)
- Proof of permanent disability (e.g. membership card of a disability organisation / persons with disabilities concession card / doctor's memo) (maximum of 5MB in JPG / JPEG format)
- Education and skills certifications (maximum 5 supporting certifications, maximum of 5MB each file in JPG / JPEG format)
- Testimonials that acknowledge nominee's character, and the promise and potential shown in an area of talent / skills / abilities (maximum 5 supporting testimonials, maximum of 5MB each file in JPG / JPEG format)

Resilience
Please provide concrete examples demonstrating the nominee's courage and perseverance in overcoming life's challenges since the onset of his / her disabilities.Community Service
Please detail how the nominee's service to others positively impacts the community, incorporating any participation in school groups / activities if the nominee is currently a student.Performance & Potential
Please elaborate how the nominee has demonstrated both aptitude and dedicated effort in developing or honing his / her talent(s).Champion for Inclusion
Awardees will be recognised as a "Champion for Inclusion" for the disability sector. Please share at least one example of how the nominee will be able to contribute to the sector should he / she be chosen as a Goh Chok Tong Enable Awards Awardee.

3: Nominator’s Acknowledgement
- I declare that all information and supporting documents submitting for this nomination are true, accurate and complete to the best of my knowledge.
- I acknowledge and agree that SG Enable, as the administrator of the Goh Chok Tong Enable Awards, reserves the right to verify any information provided with relevant individuals, organisations, and / or public sources for the purposes of assessment and due diligence.
- I understand that the submission of any false, misleading, or materially inaccurate information may result in disqualification at any stage of the awards process, or revocation of the award after conferral.
- I will promptly notify SG Enable of any changes to the submitted information before Tuesday, 23 June 2026.
- I consent to SG Enable contacting referees and relevant third parties for verification purposes.
- I have obtained my nominee’s consent to share his / her personal data for the purpose of this nomination.
- I acknowledge that the Personal Data I provided in this form may be processed by SG Enable, Mediacorp and other relevant entities for the administration of the Awards.
